Participate in the largest health surveillance on flight attendant health to date in the U.S.  We want to know what health conditions are more prevalent (or less prevalent) in flight attendants.

We found out from you in 2007 that respiratory disease, sleep disorders, fatigue, depression and heart disease were significantly elevated among flight attendants compared to the general U.S. population (http://www.ehjournal.net/content/13/1/13).

Now we are back for a follow-up study in 2014, and we've improved our questionnaire based on your feedback.  We need your help to put FA health on the map.  If you take the questionnaire by Nov. 7th, you will be automatically entered into a raffle for an iPad Air.

To take the survey and be entered into the drawing, please visit the link below. It takes 20 minutes of your time that will amount to a lifetime of knowledge--
