Onboard Service and Maintenance have developed new cabin maintenance forms that will appear on airplanes beginning mid-November.

Their objective, according to Onboard Updates, was to develop forms that would encourage flight attendants to log more defective cabin maintenance items.  This means "friendly and simple".  The provisioning location of the new form will not change.

Other info:

United provisions two types of cabin maintenance forms, one for B777 airplanes and another for non-B777 airplanes. Highlights of the new cabin maintenance forms include:

  • Reduction in the number of categories on the form from 344 items to less than 80, based on an analysis of the most commonly reported items.

  • Increased font size to enable easier reading, especially in dimmed cabins.

  • Dramatically improved item categorization (e.g., inoperative reading lights were moved from the “seat” section on the old form, to the “lights” section on the new form).

Maintenance must evaluate every “log” item flight attendants report prior to the airplane's release. If a defective item is irreparable, the write-up will be documented as a “carry-forward” or “deferral.” Repairs will occur as soon as possible given aircraft ground times, maintenance resources and parts availability. In addition, flight attendant satisfaction, in terms of workability of equipment on board, is very important. Maintenance personnel are reaching out to flight attendants for help in identifying all defective equipment, which has a direct impact on their meeting customer Marketrak workability goals.
