Report a Contract Interpretation (read Violation) issue via the Flight Attendant Contract Tracking System (FACTS).

Type: DIS*411

You'll get a response similar to the following:

FILE# Enter your file number. Include leading zeros for file numbers with less than six digits (e.g., 000123).
ID# Enter the four-digit ID number.
POS Enter your scheduled position (e.g., FS, M, N).
ID DATE Enter the originating date of your ID. Use MMDDYYYY format.
LOCATION Enter the location where the contract violation occurred (e.g., DENSK, BOS, home).
CONTRACT REFERENCE Enter contract reference (i.e., 8.J.) or topic (i.e., 8-in-24 legality). 
COMMENTS Enter detailed comments about the situation.

What happens next?

After you submit this DIS*411 form, the following three events will occur:

  • You'll receive an e-note confirming receipt of your DIS*411 report, generally within two or three business days.
  • Next, your report will be routed to an appropriate subject matter "expert" for research.  Don't ask me what this means!
  • Finally, the subject matter "expert" will contact you, in most cases by telephone, to provide feedback.
